PHP Developer needed to create dynamic web form

I need a web form developed that adjusts following user input.

Description:

I am in need of having a web form designed that allows for different options upon submission based on user input. This is for a PPC landing page.

 

I am collecting financial information and depending on certain user input the form needs to redirect the user in the end to a different offer. There is a total of 4 variables that can change the outcome of which financial offer I can direct the user to. For one offer the collected data needs to be added to a URL string to prepopulate the next page (sample of URL will be provided). There is a total of 4 different redirect options that the form needs to cover.

 

I prefer a solution in PHP5, but are open for other options as needed.
